and non-urgent services for Campus Maintenance, Hospital Maintenance,
Energy Management, and Support Services. Help us to help you by
providing all of the information requested in the spaces provided below.
If your browser does not support forms, please use this mail link,
using the content of this form as a guideline for the information we need
to service your request.

Important Note: For Urgent or Emergency Maintenance Requests, please do
not use this form, instead call the Campus Maintenance Dispatchers at
#4-5353 or the Hospital Maintenance Dispatchers at #4-6181.

These electronic on-line requests are only read and monitored during normal
business hours between 7:30a.m. - 3:30p.m. Those received after hours and
on weekends will not be read until the next normal business day.
